will have to download a form, which is called as the Form 6 for the

you have to take a print out of it. You are also required to upload all the
documents that you have like that of the birth certificate and also your

sent to the CEO of the state, they will register it and then verification will
take place in which these officers will visit your place and also will verify
your documents. They will also provide you with a receipt and then you will be
